The group estimated that between 2,000 and 10,000 European children already had the type of diabetes usually diagnosed in middle age.

As the estimates were unveiled, a statement was delivered to the conference on behalf of EU Commissioner for Health and Consumer Protection Markos Kyprianou.

He outlined plans for a Europewide code of conduct to rein in the marketing of unhealthy food to children and broader policy initiatives across agriculture, education and transport that address the obesity problem.

The European Union would publish a new strategy on diet and exercise before the end of the year and submit the document to public consultation with the food industry, anti-obesity activists and others to shape a final plan by the end of 2006, Mr Kyprianou said.

The move echoes unprecedented steps taken last year by the World Health Organisation, which launched a global strategy on diet and physical activity after health ministers from around the world approved the plan.

Akiko Abe has barely seen her 25-year-old son in six years, yet they live in the same small house. He leaves his room only when he's sure his parents are out or asleep, she said. She can tell when he has used the kitchen, and she knows he goes to the living room to watch television and use the computer at night.

She has waited patiently for him to tire of his isolation, sometimes standing outside his door and talking, to herself as much as to him. But, afraid that many more years would pass like this, she finally approached an organization that works with shut-ins by making home visits. "It will be difficult, because he won't open his door," she said quietly.

As many as a million Japanese -- most of them young men -- are considered shut-ins, either literally cloistered in their rooms or refusing to work and avoiding all social contact for periods ranging from six months to more than 10 years. Forty-one percent live reclusively for one to five years, according to a government survey. Some shut-ins suffer from such illnesses as depression, agoraphobia or schizophrenia. But experts say the vast majority shut themselves up at home for six months or more without showing any other signs of neurological or psychiatric disorder. The seriousness of the problem has increased dramatically over the past decade as Japan's economy has slid into recession, bringing record unemployment rates and little job security as companies restructure or go bankrupt.

Psychologists and other mental health experts here say that Japan has the biggest problem of this type in the world, and that it is growing. They give a long list of reasons why young men are dropping out of society, including a declining birthrate, which means there are more families with only one son on whom they place all their hopes in this patrilineal society. Also, boys grow up without male role models because their fathers are working all the time. In an attempt to get their son to communicate with them, Abe and her husband have decided that from now on, they are not going to slip an envelope under his door with his '400 monthly allowance. Shut-ins -- 70 to 80 percent of whom are men -- often sleep much of the day and are up all night, watching television, using the Internet and popping out to the 24-hour convenience stores that are located in most neighborhoods and sell all kinds of microwaveable packaged meals. Japan's convenience store culture caters to the solitary life -- providing everything for the person eating alone, living alone.

"In Japan, it's easy for anybody to live with walls around themselves," said Seiei Muto of the Tokyo Mental Health Academy. "And with the number of children declining, you play alone, eat alone, study alone."

Muto and other mental health workers talk about the decline of communication skills, the increasing anonymity of urban Japan and the collapse of a cooperative society. "If a child is walking down the street, it would be rare for someone to ask the child, 'Where are you going?' " Muto said. Others say the problem has deep historical and cultural roots. "Japan is a rich country, but we have no identity, no confidence, no ability to communicate with others," said Tadashi Yamazoe, a professor of clinical psychology at Kyoto Gakuen University. "Japanese have a passive personality." But most people say it is a modern phenomenon, evidence of a great generation gap between those who built Japan's postwar economic success, and their children, who cannot expect lifetime employment in today's weak economy and say they do not want it anyway.

"In Japan there has been only one path, and today an increasing number of people are not on it," said Noki Futagami, who began the nonprofit New Start Foundation to work with shut-ins. "It's easy to say that academic background is not everything. But the parents cannot suggest another path because they don't know one." The existence of large numbers of shut-ins in many ways encapsulates the social problems of modern Japan and the wrenching period it is now going through. The Japanese word for the phenomenon -- hikikomori -- translates as withdrawal, and it is becoming increasingly familiar. It is the subject of television documentaries and newspaper and magazine articles.

Many adult shut-ins start as school dropouts. For a country obsessed with education, there is a surprisingly high number of dropouts. A record 134,000 elementary and junior high students were absent from school for at least 30 straight days during the 2000-01 school year, more than twice the number 10 years ago.

Abe said her son's school years were normal, but in high school he failed the university entrance exam. That is not unusual; most who fail study for another year and try again. Abe's son said he was going to study on his own instead of enrolling in a cram school, and that began his withdrawal.

The family has tried to keep the problem hidden, not even talking about it to relatives, much less neighbors.

But Futagami said this means the family is shutting itself in as well, making the problem worse. "There are things parents can and cannot do," he said. "They should be more open and get help from others, nurture social ties. I regard this as an illness stemming from society. Nobody helps these people, so they accumulate." In a few recent cases, socially withdrawn young men have committed shocking crimes, including a 27-year-old who kidnapped a 9-year-old girl in 1990 and kept her in his room for nine years. His mother, who lived downstairs, was never permitted to enter his room.

"In America, the child's room belongs to the parents and is seen as being rented out to the kid," noted one of the actors appearing in a new play on shut-ins. "The child can be displaced for guests." This is a remarkable concept in Japan, where the norm is that teens or young adults can forbid their parents from entering their rooms.

As the problem gets more national attention, parent support groups, counseling centers and mental health clinics have geared up to help families. Home visits over the course of months and sometimes years bring many people out of their rooms. But finding a job after having spent several years as a shut-in is extremely difficult. To provide work experience, Futagami's New Start organization runs a welfare center for the elderly, a restaurant and coffee shop.

Takeshi Watanabe, a counselor with the Tokyo Mental Health Academy, and Yasutaka Masuko, 28, seem like brothers. For 10 years Watanabe visited Masuko once a week at the home Masuko refused to leave. Masuko said he doesn't remember anything specific causing him to drop out of school during his second year of junior high. "Maybe I was feeling pressure," he said. For a while he became physically ill when people came to see him.

But Watanabe's steady visits, their shared interest in music and eventually Masuko's purchase of a computer slowly convinced Masuko that he could go out. The turning point was soccer. He wanted so badly to go to the games of his favorite team -- an interest encouraged by Watanabe -- that he bought a season ticket, and before the first game practiced going outside.

"For night games I went early in the morning to get a good seat," he said. "I made friends because I was in the same place every game." Masuko has taken other big steps. He got his high school degree through a correspondence course and is now enrolled at Nihon University, majoring in philosophy and education. He said there are many other former shut-ins there, and they often talk. He also found a part-time job at a loan collection company. An understanding society is critical to dealing with the problem, Watanabe said. The mental health clinic in a Tokyo suburb where he works has cultivated about a dozen business establishments in the immediate neighborhood, where they have introduced themselves and the young men who come by. "We wanted them to understand we are not a cult," Watanabe said. At the bike shop, coffee shop and 7-Eleven, people started to talk to them, started to say, "Hi, how's it going?" They got emotional support from the neighborhood and some shopkeepers hired them to work two to three hours per week, he explained. "Many people feel nostalgic about Japanese traditions and the warmth that is harder to find today," Watanabe said.

Puerto Rico warns of health risk from fuel fire

SAN JUAN, Puerto Rico – Thousands of people living on the outskirts of a burning fuel-storage site in Puerto Rico were urged to relocate Saturday to avoid toxic smoke still billowing from the fire.

More than 1,500 people were evacuated after Friday's earthshaking explosion at the Caribbean Petroleum Corp. in the suburb of Bayamon, just west of the capital of San Juan.

Only a few people were slightly injured by the blast, which broke windows, tore doors off their hinges and shook the ground. But authorities are now concerned about those downwind of the fire, with breezes pushing smoke toward more populated neighborhoods.

Authorities urged residents — especially those with respiratory problems — who live near the site to temporarily move elsewhere until the fire is extinguished and the smoke brought under control.

"To those people with respiratory problems, our recommendation is that you not wait," Gov. Luis Fortuno said.

Several people have sought medical help for respiratory distress, and one firefighter was treated Saturday for exposure to high temperatures, Fortuno said. Those who were evacuated and an additional 530 people who sought shelter away from home have not returned.

Fortuno said a large stadium has been prepared to accommodate some 30,000 people who live in the area if necessary.

Crews were monitoring the situation and anticipated more evacuations if weather worsened.

"The smoke is extremely toxic," said Jose Bartolomei, a state epidemiologist who monitors asthma. "An asthmatic patient will definitely hyper-react to this."

Adding to the danger is the presence of invisible gases such as carbon monoxide and sulfur, said Luis Antonio Ocasio, spokesman for the island's Environmental Quality Board.

Local officials, along with a New Jersey crew from the U.S. Environmental Protection Agency, are monitoring the air with handheld devices and probing whether any fuel spilled into the nearby ocean, he said.

The cause of the explosion is still under investigation. Fifteen of 40 fuel tanks caught fire, and five were still burning Saturday afternoon, Fortuno said.

He has declared a state of emergency for five municipalities, including San Juan and Bayamon, but said initial estimated damages of $6.4 million are not sufficient to receive federal funds.

Gulf spokesman Eric Guzman said Saturday that the company has hired an independent crew to help quell the fire and that it is cooperating with authorities. He said he did not know how much fuel is stored in the tanks.

WASHINGTON – President Barack Obama declared the swine flu outbreak a national emergency, giving his health chief the power to let hospitals move emergency rooms offsite to speed treatment and protect noninfected patients.

The declaration, signed Friday night and announced Saturday, comes with the disease more prevalent than ever in the country and production delays undercutting the government's initial, optimistic estimates that as many as 120 million doses of the vaccine could be available by mid-October.

Health authorities say more than 1,000 people in the United States, including almost 100 children, have died from the strain of flu known as H1N1, and 46 states have widespread flu activity. So far only 11 million doses have gone out to health departments, doctor's offices and other providers, according to the Centers for Disease Control and Prevention officials.

Administration officials said the declaration was a pre-emptive move designed to make decisions easier when they need to be made. Officials said the move was not in response to any single development.

Health and Human Services Secretary Kathleen Sebelius now has authority to bypass federal rules when opening alternative care sites, such as offsite hospital centers at schools or community centers if hospitals seek permission.

Some hospitals have opened drive-thrus and drive-up tent clinics to screen and treat swine flu patients. The idea is to keep infectious people out of regular emergency rooms and away from other sick patients.

Hospitals could modify patient rules — for example, requiring them to give less information during a hectic time — to quicken access to treatment, with government approval, under the declaration.

It also addresses a financial question for hospitals — reimbursement for treating people at sites not typically approved. For instance, federal rules do not allow hospitals to put up treatment tents more than 250 yards away from the doors; if the tents are 300 yards or more away, typically federal dollars won't go to pay for treatment.

Administration officials said those rules might not make sense while fighting the swine flu, especially if the best piece of pavement is in the middle of a parking lot and some medical centers already are putting in place parts of their emergency plans.

The national emergency declaration was the second of two steps needed to give Sebelius extraordinary powers during a crisis.

On April 26, the administration declared swine flu a public health emergency, allowing the shipment of roughly 12 million doses of flu-fighting medications from a federal stockpile to states in case they eventually needed them. At the time, there were 20 confirmed cases in the U.S. of people recovering easily. There was no vaccine against swine flu, but the CDC had taken the initial step necessary for producing one.

"As a nation, we have prepared at all levels of government, and as individuals and communities, taking unprecedented steps to counter the emerging pandemic," Obama wrote in Saturday's declaration.

He said the pandemic keeps evolving, the rates of illness are rising rapidly in many areas and there's a potential "to overburden health care resources."

The government now hopes to have about 50 million doses of swine flu vaccine out by mid-November and 150 million in December. The flu virus has to be grown in chicken eggs, and the yield hasn't been as high as was initially hoped, officials have said.

"Many millions" of Americans have had swine flu so far, according to an estimate that CDC Director Dr. Thomas Frieden gave Friday. The government doesn't test everyone to confirm swine flu so it doesn't have an exact count. He also said there have been more than 20,000 hospitalizations.

Low-salt, low-protein diet can prevent kidney stones

A diet low in salt and meat can dramatically reduce the risk of kidney stones, according to an Italian study that could spell the end for the low-calcium diet that doctors have been recommending for years.

About 10% of the U.S. population - and a much larger percentage of men, who make up 80% of sufferers - will have at least one kidney stone sometime in their lives, and it can be excruciatingly painful. Most stones can be excreted, but in about 15% of cases, surgery or shockwave treatment to pulverize the stones is needed.

William Keane, president of the National Kidney Foundation, said he is sure the new diet "will become the gold standard."

Dr. Umberto Maggiore, explained that people on a low-calcium diet excrete less calcium in their urine but more of the other substance that combines with calcium to form most kidney stones.

In addition, salt and one protein common in meat lead to more calcium in the urine, which in turn contributes to the formation of kidney stones, Bushinsky said.

The low-salt, low-protein diet allowed 2,900 milligrams of salt per day; 2,400 is the maximum recommended by the American Heart Association. But people were allowed only about three-quarters of an ounce of meat per day, with just over an ounce of cheese or other dairy protein.

The American Foundation for Urologic Disease does not mention a low-calcium diet on its Web page about kidney stones, but does say a doctor may suggest a low-meat, low-salt diet. The National Kidney Foundation's Web page says a doctor may suggest either diet, depending on test results.

Health care for kidney stones added up to $1.8 billion in 1993, according to the National Institute of Diabetes and Digestive and Kidney Diseases. Drinking 10 to 12 big glasses of water during the day can keep stones from forming and push out those smaller than a pea.

Even a small risk to individuals can have big consequences when applied to a large population.

The study suggests that,on balance,a group of 10,000 long-term estrogen users would suffer 31 excess health crises each year(strokes,heart attacks,blood clots,breast cancers),while avoiding only 11 bone fractures and colon cancers. That's a net increase of20. If 100,000 people take up long-term HRT,they'll suffer 200 of these needless events each year. A million long-term estrogen users will experience 20,000 of them annually--which means20,000 over the course of a decade. This is no way to prevent hip fractures.

Short-term use is a different story. The study didn't turn up unacceptable risks among womentreated for less than five years,and experts agree there is still no better treatment for the hot flashes,mood swings and insomnia that many women experience around the time of menopause.

Can Your Weight Keep You From Getting a Job?

Obesity is one of the nation's greatest public health problems. According to the Center for Disease Control, an estimated 64 percent of U.S. adults are either overweight or obese. Studies show that overweight Americans suffer medical ailments more often than their slimmer peers – from diabetes and hypertension to degenerative conditions like arthritis and many types of cancer.

And if that news isn't enough to get you on the treadmill chew on this: Studies also show that out-of-shape Americans earn less on average than their co-workers do. Not only can being fit cost you less in medical bills and insurance, it can earn you more on the job.

Al Mientus, an executive recruiter for Pinnacle Partnership, says employers are "significantly less inclined" to hire the overweight. "It has to do with an employer's perception of work ethic," he says. "Employers believe that if you were hard-working and ambitious you would stay in shape."

The National Association to Advance Fat Acceptance (NAAFA) states that only one in 11 top male executives is overweight. The message is clear: Those who are fit enough to climb are more likely to make it to the top of the ladder.

Academic studies provide further proof that being in shape can boost your earning potential. A University of Michigan study reported that the total net worth of moderately to severely obese women is as much as 60 percent below average. And a Cornell study by John Cawley, professor of policy analysis, found that overweight Caucasian women earn 9 percent less than those who are slim.

So will being out of shape keep you from getting a job? Another Cawley study indicates for mid-level and entry level jobs, "...there is no effect of weight on the probability of employment or sector of occupation." But carrying some extra pounds can hurt your chances of climbing the ladder. "At the executive level, being overweight will hinder your chances of employment," Mientus says.

If building a better body will help you build a better career, what steps should you take for a job-boosting physique? Mientus tells his clients to approach body improvement like they approach improving their job skills. Here are his suggested solutions:

Define your goals: "Just like identifying that you have to improve certain work skills, you must also identify what health skills you need to improve," Mientus says. Whether it's altering your diet or getting more aerobic exercise, even building muscle mass, it's important to set goals for improvement.

Schedule healthy activities: "You have to make appointments for exercise and eating right, just like you schedule time in your day for meetings or training that will boost your skills or network," Mientus says.

Find a mentor: "The best mentor is one that is at your workplace with you – someone who fits exercise into their work day," Mientus says. If you can't find someone at work who can lead by example, a spouse or friend is also a good choice. "Someone who will see you every day to help remind and guide you is very helpful," he adds.

Dress the part: Once you start to get in better shape, choose clothes that tastefully show off your new physique. "Fit people naturally look more confident, but good clothes that fit well accentuate that confident look," Mientus notes.

So on your next lunch break, grab a workout with the sales manager and then join her for an Evian and chicken quesadilla (hold the sour cream). Your thinner body, and fatter wallet, will thank you!

The study, published this week in The Lancet medical journal, also found that it doesn't seem to matter what people drink - the effect is the same.

Experts say moderation - between one and three drinks a day - is the key.

The adverse effect of excess alcohol is beyond question. Besides destroying the liver, several studies have shown that excessive drinking can be toxic to the brain. Alcoholics can end up with a shrunken brain, which is linked to dementia. There is even a medical condition called alcoholic dementia

Scientists at Erasmus University in Rotterdam, the Netherlands, conducted a six-year study of 5,395 people aged 55 and over who did not have signs of dementia.

They were asked whether they ever drank alcohol. Those who said yes were quizzed on how often they drank and details on their consumption of specific drinks such as wine, beer, spirits and fortified wine such as sherry and port.

The men mostly drank beer and liquor, while women preferred wine and fortified wine.

The researchers also checked whether participants' drinking habits had changed over the preceding five years or whether they had engaged in binge drinking - more than six drinks in one day.

Everyone was categorized according to how much they drank. Four or more glasses of alcohol per day were considered heavy drinking.

By the end of the study in 1999, 197 of the participants had developed Alzheimer's or another form of dementia. Those who fared best were people who drank between one and three drinks a day. They had a 42 percent lower risk of developing dementia than the nondrinkers.

Those who weren't daily drinkers but had more than one drink per week had a 25 percent lower risk and those who drank less than a glass a week were 18 percent less likely than nondrinkers to develop dementia. Drinking heavily did not affect overall dementia.

Heavy drinkers, who numbered 165 - mostly men - were 1 1/2times more likely to get vascular dementia and slightly more likely than nondrinkers of ending up with Alzheimer's.

Researchers suggested the blood-thinning and cholesterol-lowering properties of ethanol in alcohol may ward off dementia, which is often caused by a blood vessel problem.

Another possibility, the study speculated, is that low levels of alcohol could stimulate the release acetylcholine, a brain chemical believed to facilitate learning and memory.
